Patient Questionnaire is designed to replace the new-patient forms, health history update, and other forms used in your office to collect information from patients. With Patient Questionnaire, you can create a set of questions for your patients to answer. For those offices moving toward the "paperless office," Patient Questionnaire is a way to computerize information that otherwise would be stored in a paper chart. Best of all, when the information needs to be reviewed for accuracy, a new copy with the previous answers can be generated; the patient only has to modify those items that have changed since the last visit.
When a patient arrives for an appointment, you can do one of the following:
Print a questionnaire, have the patient complete the printed form, and then enter the responses into Dentrix Enterprise.
Go through the questions with the patient and enter the responses into Dentrix Enterprise as you go.
Note: Accessing Patient Questionnaire requires the "Patient Questionnaire Open" security right.
To open Patient Questionnaire
Do one of the following:
With a patient selected in Family File, Ledger, or Chart, click the Patient Questionnaire button on the toolbar.
If the patient has questionnaires, the button has a red check mark.
If the patient does not have questionnaires, there is not a red check mark.
With a patient selected in Document Center or Treatment Planner, click the Questionnaires button on the toolbar.
If the patient has questionnaires, the button has a red check mark.
If the patient does not have questionnaires, there is not a red check mark.
With a patient's appointment selected in Appointment Book, click the Patient Questionnaire button on the toolbar.
If the patient has questionnaires, the button has a red check mark.
If the patient does not have questionnaires, there is not a red check mark.
With a guarantor selected in Collections Manager, click the Patient Questionnaire button on the toolbar.
If the patient has questionnaires, the button has a red check mark.
If the patient does not have questionnaires, there is not a red check mark.
With a patient selected in Family File, Ledger, or Chart, on the File menu, point to Switch To, and then click Patient Questionnaire.
With a patient selected in Document Center or Treatment Planner, on the File menu, point to Switch To, and then click Questionnaires.
With a patient's appointment selected in Appointment Book, on the File menu, point to Switch To, and then click Patient Questionnaire.
With a guarantor selected in Collections Manager, on the File menu, click Patient Questionnaire.
If applicable, do one of the following:
If the Password - Patient Questionnaire Open dialog box appears, user verification has been assigned to the "Patient Questionnaire Open" task. Do one of the following:
If your user account has been granted the "Patient Questionnaire Open" security right, enter your credentials in the User ID and Password boxes, and then click OK.
If your user account has not been granted the "Patient Questionnaire Open" security right, have a user with permission temporarily override this restriction so you can access Patient Questionnaire this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Patient Questionnaire Open" security right. To have a user with permission temporarily override this restriction so you can access Patient Questionnaire this one time, do the following:
On the message, click Yes.
In the Password - Patient Questionnaire Open dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
The Patient Questionnaire History dialog box appears and displays any existing questionnaires that are attached to the patient's record.
